Restroom cleaning log template
Restrooms are where most client complaints start, so this is the log most supervisors check first. Our free template is a one-page sheet with a row per visit and the items a supervisor actually looks at. It is our own original layout. No email required.
Free printable PDF. For your own business use. Please do not resell or repost it.

What each row covers
- Date, time, cleaner initials, and restroom location (restroom 1, restroom 2, lobby level, etc.)
- Fixtures: toilets and urinals cleaned inside and out, including bases and the floor behind them
- Sinks and faucets: wiped, polished, no water spots
- Mirrors: streak-free at eye level
- Dispensers: soap, paper towels, toilet paper checked and refilled
- Floors: mopped from the far corner to the door, mats lifted and reset
- Trash: emptied, liner replaced, stall doors wiped
- Supervisor sign-off column for the weekly walk-through
How to use it
Post one sheet per restroom and change it weekly, or keep a folder of sheets at the site office. The cleaner initials each visit when the work is done. The supervisor initials the sign-off column on the walk-through and writes any fix directly on the sheet with a date. If a client ever asks when a restroom was last done, this sheet is the answer.
Restrooms are also where photo rules matter most: no photos inside restrooms, locker rooms, or any space where people expect privacy. The record for those spaces is checklist plus timestamp only. Read the full photo and privacy rules.
From paper to proof
The same rows run digitally in MopLogic: the cleaner checks each item on the phone with a timestamp, the supervisor signs off in the same app, and the monthly proof report shows the client how many restroom visits were logged and every issue closed. See how it works.
